Care Navigation

The Care Navigation service can support people with social care needs who may not be eligible for help from Adult Social Care services.

The service is available to people who live in Plymouth with any disability aged 18 to 64.

Care Navigation can help with the following:-

Providing information, advice or support to access other services

• Assessments of need and help with self-assessments
• Putting a support plan together and helping to organise the plan by signposting or referring on to appropriate services
• Maximising income, helping with disability benefit applications and supporting access to debt advice
• Providing advice about services that can help people return to work and helping them to make links with those services
• Providing advice about services that help people keep their accommodation and avoid eviction and helping to make links with those services
• Assisting with finding and participating in training, leisure and social opportunities
• Helping to find the support, advice and equipment needed for independence
• Developing the confidence and ability to increase choice and control
