Patient Community Support and Advice
Health and social care services in the future will focus on people
having choice and control over their own health and care. The vision
is of people who use services, empowered with advice, support
and information, having choice about the services they want, being
able to take more responsibility for their health and their lives and a
more active role in managing their own care if this is what they want. (The Common Core Principles to Support Self Care Guidance, DOH May 2008)
Plymouth Guild in Partnership with the Plymouth teaching Primary Care (PtPCT) has developed a Patient Support Team to give patients in the community and using the PCT community Services independent information, advice, advocacy and support. The team can...
- Provide help, advice and information to patients their carers and relatives
- Support you in managing your long term health problems by becoming an Expert Patient
- Offer a confidential and independent community advocacy service to people accessing mental health care services in Plymouth who may have questions or concerns about their treatment.
